The owner will be responsible for all transport to and from the Welfare vet. Price is subject to change if these costs to us are increased by veterinarians. Additional costs for surgery for any other conditions will be at the owner’s expense.ADOPTION CONTRACTConditions of adoption(Required) I agree to all the terms:You send Jacks Rescue a copy of each vaccination. In puppies for the first three months and for adult dog, yearly. The outside area where the dog will be kept must remain entirely and securely fenced off and the gates to this area will be kept closed and locked at all times. If the gate/s are to be kept open for a particular reason, the dog is to be placed on a leash under supervision. The enclosure will have fencing/walls of sufficient height so that the dog cannot jump or scale or jump the walls or fences. In the case of puppies/kittens and very old dogs/cats(including partially sighted dogs/puppies/cats/kittens), the swimming pool will remain completely and securely fenced off so that dogs/cats cannot enter the pool area. In the case of healthy adult dogs, the dog is to be shown how to get to the steps leading out of the pool, by going into the pool with the dog several times at the deep end and showing it how to swim to the steps if it should fall in. Dogs' tails will not be docked and ears will not be cropped. Cats will not be declawed. Legal action will be taken against the owner if these mutilations are done to dogs and cats adopted from Jack’s Rescue. Dogs will be exercised regularly before they eat. Exercising your dog on a full stomach causes torsion, whereby the stomach turns over and the blood supply is cut off. The dog's stomach blows up. The dog is in great pain and could die without immediate surgery. When adopting a cat all windows and doors must stay closed for 10 days in the area where the cat is kept until it has settled in so that it will become familiar with its surroundings. Later the cat should be introduced to the rest of the house with all windows and doors shut. It is recommended that a cat harness and leash be used when taking the cat outdoors, especially if there are busy streets nearby Long-haired animals, such as Maltese poodles and Persian cats must be groomed regularly and their hair kept free of knots at all times. Dogs will not be chained under any circumstances or be subjected to any psychological or physical abuse. The dog/cat will be provided with an identity disc for its collar. This must never be removed. The dog/cat will also be microchipped to ensure that if the dog should get out of your grounds, the owner can be identified immediately. Cat collars must be elasticized. The dog /cat will be kept as a pet and treated as part of the family. All dogs and cats must sleep indoors in your home. Vaccinations and de-worming must be kept up to date and the pet must be free of parasites such as fleas and ticks. The pet will be taken immediately to a registered vet, should it become ill. The pet will be kept safe and secure indoors during times when it may be subjected to unusual stress such as Guy Fawkes, New Year, or if you were to move house. If the pet gets lost, Jack’s Rescue must be notified immediately. Appropriate action must be taken to recover the cat, such as contacting all animal agencies as follows: TEARS 785 4482, SPCA 7004140, The Animal Welfare Society 6922626, AACL 5346426, DARG 7900383. Adverts under “lost” should be placed in community newspapers 4884892 and the daily press 4884891 Argus and Cape Times 4062222 (Die Burger). Flyers should also be distributed at Taxi Ranks with a reward offered “no questions asked” basis and similar ads to be placed in supermarkets, shopping centers, etc. If you are adopting a puppy/kitten that is too young to be sterilized, the sterilization fee will be paid for in advance as part of the adoption fee. The puppy/ kitten will be recalled for sterilization at 5½ (female) and 6 (male) months. You will advise as to which veterinarian to use. If you wish to use your veterinarian, you will be responsible for the full sterilization fee which your veterinarian charges, and no refunds will be made by Jack’s Rescue. The sterilization must still take place at the same age as stipulated by adopting a pet and proof of sterilization must be forwarded to adopt a pet immediately after the pet has been sterilized. If the animal is not sterilized within the stipulated time, I agree that it will be removed for rehoming by Jack’s Rescue. If the pet displays any type of illness that requires any veterinary attention for the pre-existing condition within the first seven days after adoption, the animal must be returned to Jack’s Rescue for veterinary treatment by associated veterinarians. If however, the owner elects to use his or her veterinarian, Jack’s Rescue will not refund any costs incurred. Jack’s Rescue is not responsible for any veterinary fees of whatever nature whatsoever after the seven-day period has expired. Jack’s Rescue is not responsible for any injuries or damage of any nature whatsoever, howsoever caused which may be inflicted on the new owner, His or Her family or friends or acquaintances or strangers, of his or her existing pets or property, or pets and property belonging to any other persons by any of the adopt-a-pet pets available for adoption either when viewing them or after adoption. Viewing, touching, walking, and adopting pets from Jack’s Rescue are done entirely at your own risk. Jack’s Rescue reserves the right to conduct follow-up visits to the adoptive home to ensure compliance with the conditions of adoption. Should Jack’s Rescue find during follow-up visits, that one of these conditions is not adhered to, the owner will, on request, return the pet and any payment/donation made in respect of the pet will not be refunded. The adoption fee will be refunded for any pet that is returned within 14 days from the date of adoption. The pet will reside at the owner's address given on the reverse side of this form and Jack’s Rescue will be notified of any change in this address. Please be advised that Jack's Rescue cannot be held liable for any injuries or damages caused by the adopted pet. By adopting a pet from us, you agree to assume all responsibility for the care and actions of the animal. IMPORTANT: If the owner is no longer able to keep the pet for any reason, ownership reverts to Jack’s Rescue and the pet must be returned immediately to Jack’s Rescue. The pet may not be disposed of by gift, sale, or any other means, whether to any animal welfare organization other than Jack’s Rescue, private individuals, veterinarians, etc as this will result in legal action being taken against the owner. Please remember, if the animal is not the right one for you, Jack’s Rescue welcomes back the animal at any time and it will not be put down.
